Intake forms are required 24 hours prior to your scheduled appointment.  They are available to you below, and in your confirmation email.  They are always required if it is your first time receiving that type of massage with us, for each new pregnancy, and for each new postpartum period.  Please fill out to the best of your knowledge and submit with adequate time for your therapist to prepare for your appointment.   Thank you!
